I just don't get it.
Ok.. how is it that a guy (I'm not saying all...) can tell the world he's "with" someone.. and then keep that person only as an option and not make them a priority?

It helps to have a good feeling about yourself so you don't feel you need to be his priority. And understand that a womens sense of priorty is a whole lot different then a mans. The football game is a priority. His friends are a priority.
A woman is the option, he played the game, he chased you, caught you, he's won the prize! So what more does he need to do? It seems like once they have you they think they don't have to make the effort to keep you.
At a guess, some people are just users. Others just don't really have a healthy concept of what it means to be in a relationship, whether because of their relationship role models, social education, or just because they are completely self-centered.
